General Information

There is a one-time enrollment or initiation fee of $79.00 per pet.

If that pet is examined once a year, the enrollment remains in effect. There is a regular fee for that annual examination.

If this is the first time we have seen the pet, the examination fee AND the enrollment fee are due at the same time.

If we have seen your pet in the last 6 months and you wish to enroll in the program, your pet does not need another examination to get started. This pet will still have the requirement of an examination within 12 months of the last examination.

This program can save considerable amounts of money.

Puppy or kitten boosters

Annual or 3 year Rabies vaccines

Annual or 3 year dog or cat distemper combinations

Leptospirosis vaccines for dogs

Annual or 3 year vaccines for feline leukemia

Bordetella / Kennel cough every 6 or 12 months depending on the kennel requirements

Any other vaccines are NOT free.

Other services and products are NOT free. This includes but is not limited to laboratory tests, x-rays, surgery, dentistry, and medications.

The plan is non transferable, non-refundable, and is not exchangeable for any reason

Common Questions

Q: If I purchase the program from you, can I use it at any other veterinarian?

A: No, the Free Vaccines for Life Program is exclusively offered at Family Pet Hospital.

Q: If I have more than one animal, do I get a discount on any other animals?

A: No, we do not offer any discounts to go along with this program. It is a

one- time fee per pet, per owner.

Q: Does my Senior Citizen discount apply?

A: No, the discount applies to the examination by the doctor and does NOT apply to the program enrollment fee of $79.00.

Q: Can I transfer the policy to another pet?

A: No, the policy is for one pet.

Q: If my pet dies or runs away, after purchasing the policy can I get a refund?

A: No, we do not give refunds after purchase for any reason.

Q: If I give away my pet, will the new owner be able to use the policy?

A: No. If you give your pet away then the new owners will have to purchase their own policy.

Q: What happens if I do not make it in to see the doctor before my year passes?

A: If you do not come in to see the doctor within 1 month (30 days) of its due date then your policy becomes void. You will have to purchase any vaccines from that point on, or if the plan is still being offered you may enroll again.
